It's not supposed to happen, but it happened to me twice when doing the infiltrate Ryujin quest. And a separate time when infiltrating Generdyne. I'm sneaking, nobody knows I'm there, and there's this random guard in the vent, totally blowing my cover. This isn't supposed to happen, and yet it happened to me on three separate occasions. And this is "new as in pressing new game" new games.

Sadly, this appears to be random. Some games it never happens, and other games it happens. Frankly, I don't know what I could be doing for this to happen, so I'm going to blame Bethesda.
