Fix frequency of UserProfileFixed updates #204

This update will a) update the fetchUserProfileFixed method in userProfileService so that it always updates UserProfileFixed (was previously only making update when isActive chnged). b) update the survey so that it calls fetchUserProfileFixed when survey is loaded.
