Default site instance structure #76

User can configure instance_path.
Database, log files, and search index receive default file names and can be found relative to the instance_path.
SQLALCHEMY_DATABASE_URI, TRAFFIC_LOG_PATH, and SEARCH_INDEX_PATH are removed from the config, because they now follow defaults.
create_site() command in cli.py initializes the instance folder and creates the expected structure
Also, add a regular log file that current_app.logger routes to

Later, I may extend this to create default site_banner.jpg, default_banner.jpg, default_thumbnail.jpg, and default_featured.jpg in static. These can be set to some simple images on init. However, that will be for a separate issue.
