ActiveX extension does not work event creating the rules

What's the URL of the page that contains a bug?

Does it work before? If not, when did you find this problem?
It is just recently that I used this extension.
What steps will reproduce the problem?
There is no step to create the problem because by default it does not work at 
all.

What is the expected output? What do you see instead?
N/A

What version of the product are you using? On what operating system? On
which version of your browser?
Production Version: 1.5.0.7
OS: Windows 7 Ultimate
Browser Version: 36.0.1985.125 m

What's your settings?
I created the rules in the extension options in this link: 
chrome-extension://lgllffgicojgllpmdbemgglaponefajn/options.html
Or you mean other settings? What are those?

Please provide any additional information below.
Actually, after install this adds-on I created the rules for my website and I 
start to test it and it does not work. The rules that I created for my website 
have the button name "Custom" not something like "Enabled" Or "Disabled". 
What's wrong? Anyway, all the existing sites listed in the extension options 
are working great.
