In Half Life 2 player reacts to damage from fire, drown and fall pain. For some reason this broken in Portal and in Portal 2.
Interest fact - in Portal and Portal 2 you still can hear drown sounds if lost oxygen and swim out of the water.
Also in Portal 2 HEV sounds just not work.
I suggest cvar sv_hl2_player_pain_sounds_enabled that will toggle beetwen Portal player pain sounds and Half Life 2 player pain sounds, 1 - enabled, 0 - disabled, 2 - combined. Main differences between Portal and Half Life 2 pain sounds mechanic is also that Portal player reacts on crush damage (when cube fall on player) and barnacle eat, this not happen with Half Life 2 player.
Cvar should not disable HEV voice, HEV voice should never disabling or have separate cvar that can toggle it.
