Figure out best way to generate an index per-jurisdiction #4

Ultimately we want ma.courtformsonline.org, il.courtformsonline.org

But we need to make it clear that the content belongs to the author of the forms, not our website.

We can add a jurisdiction to formSources.config.ts, and assume that jurisdiction is defined per-server.

Sounds like we don't want subdomains, just using a route for each state /ma e.g. would be enough

Out of scope: Landing page to display all jurisdictions and would be confusing since we're only tackling Massachusetts.
