For each 'add-field' issue, update the DOM to Bootstrap 5 DOM #160

Like SuffolkLITLab/docassemble-ALKilnSetup#59 to make sure we update the recorded DOM for Bootstrap 5.

  1. Start a new comment.
  2. Add the line 'Old DOM:'
  3. Paste the old html as html code
  4. Save the comment
  5. Edit the first comment
  6. Delete the old DOM
  7. Add "Bootstrap 5 DOM:" on a new line
  8. Paste the new DOM as html code

See https://github.com/SuffolkLITLab/ALKiln/issues/389#issuecomment-975640010 for the changes we found we needed to implement. Will try to update these as time goes on, but this is very boring and it's not quite clear how useful this will actually be moving forward.

Actually, we'll retire this until we see a clear need for it.
