Cleanup the add/label field interface #901

  1. Makes use of Docassemble's new grid feature for showing fields side by side to improve the appearance of the labeling page.
  2. Adds an "optional" flag for fields
  3. Fixed issue where the author wasn't able to add choices to a multiselect, radio, dropdown, or combobox field from the "add a new field" menu.

There's not a rush for these new features. I'll plan to demo on a Monday call.
