Triage and Transfer open issues to ALAffidavit

Most of the open issues and user feedback here is still applicable to ALAffidavitOfIndigency. Should give each issue a descriptive title, an actionable change to make, and transfer it to that repo. If it's not actionable, but still represents some general confusion about a screen, should label it with "user feedback" so we can easily aggregate such feedback later when triaging.

Finally done! All that is left to migrate other interviews off this and onto the ALAffidavit, and archive this repo, as described in #43
