Consider disabling the screen reader by default #835

There are a number of UX problems with the screen reader, and in a discussion we decided it may be right to stop supporting it now.

E.g., #621 #664

Some ideas:

Check a global configuration option that we default to being True (to provide a transition time)
add an intermediate value, like al_speak_text, so that the speak_text option can easily be turned off on an individual interview
