Figure out how to safely amend the "contact_methods" question - it's not relevant in post-covid world #860

  1. It asks for "other contact information" -- we don't use this in most templates so it's irrelevant
  2. It has all fields optional and requires answering 1 -- that's not necessarily right for most forms
