adds California, Indiana, and Louisiana as selections
- don't show Louisiana for the Tyler specific interviews
only shows which jurisdictions are enabled on the proxy server
for unauth, make sure that the API key we have is correct, otherwise, there's nothing we can do.

Did some manual tests with Appearance, looks okay still. Going to merge
