Add metadata to help with courtformsonline.org

@jtmst this is an example complete metadata block. It will come over as JSON in a mirror of the YAML format. This is the first form to use most of these keys--please let me know if anything looks wrong or harder to use in the new courtformsonline.org repo.

Conventions:

• Tried to use snake_case for most keys except a few keys that have an existing convention of using spaces between words. See: https://docassemble.org/docs/initial.html#metadata

Also note: we used review_date even though revision_date exists because we can check the last commit on GitHub already. Review date is meant to be the date that a person looked at the forms and law and confirmed that this interview can still be filed without a law/form update.

Did a manual test and it looks like just Kiln flakiness explains the test failure (race condition maybe where it tried to run before it finished installing)

edit: or possibly we're just not running the right version of Assembly Line on the test server?
