yesnoradio: "Are you an attorney?" If yes, ask in yesnoradio "are you filing on behalf of a person or a business?" Collect all the information (name, address, contact info) on one page rather than separate pages

After collecting info, prompt the user with “Are there additional attorneys/plaintiffs to tell us about?” → clarify that the attorney is on the plaintiff's side and not opposing counsel

"Are there any other attorneys?" question should stay. The person filing the form is always plaintiff and/or counsel for plaintiff.
