Who is the bottom address/ contact information for? #13

Is it for the user or is it for the the plaintiff or the attorney if there is one? If attorney, plaintiff information does not go anywhere

The bottom address/contact information is for the attorney who is filling it out on behalf of the defendant. This only fills if user is an attorney. Although, the form is primarily for pro se users.

for MVP user is only a pro se user; however, in class Quinten mentioned making an additional question regarding whether or not the user is an attorney and having a kick out screen if the user is an attorney
