if the form is used for all family court cases then can we have an in re or other?

currently puts together plaintiff and defendant name and asks if it is the correct case name - want yes to continue and no allow them to edit case name reconsider function?

what if they have multiple docket numbers - are the case names always the same - do we pick one?

Removed all logic and allow user to input for MVP. remains in about_the_case.yml but commented out
