This could be used in divorces or custody cases between unmarried parents. There might be other appropriate uses, but none jump out. Definitely not used in name change, which has very different service rules
Restrictions - have filed or intend to file this concurrently with a family law case; have done a "diligent search" to locate defendant
SME question - Is the user always the individual who is seeking to file for divorce and thus the plaintiff starting a new case?
Additionally, is this form used ONLY for divorce? can it be used for other means (i.e. minor changing their name) and are we restricting it for that purpose
