consider adding Google site search to list pages #2

add an input and button that sends folks to something like https://www.google.com/search?q=[query] site:suffolklitlab.org/form-explorer/form/MI/

I added this, but it won't work until the site is indexed which will be a ways off as there are not public links to the site at this time.
