Add a title for every file. If the PDF title is empty, fallback to the filename #60

If we ever use the FormFyxer in here, we have a few fallbacks there that generally work well. Using the filename is a new one though; I'm not sure what stage of processing this could happen at, but idk which is better, "(Untitled)" or the long hash in all of the file names, e.g. "d782ac01..."

Ah, good point. I was thinking we could keep the original filename around for this stage, pre-hashing.
