Remove unused 2015 rules, add new rules #17

Remove Measures from 2015 that are not used in 2016. Add new Measures for 2016.

Remove Group Measures from 2015 that are nut used in 2016. Add new Group Measures for 2016.

Skeletons++ for new individual Measures had already been created: 403-439.

Remove these Individual Measures: 33 40 81 82 163 165 166 167 168 172 173 193 194 242 349

We don't have a file for 370 for 2015??

Create Group Measure Skeletons for: Cardiovascular Prevention: 130, 204, 226, 236, 317, 438 Diabetic Retinopathy: 1, 18, 19, 117, 130, 226, 317 Multiple Chronic Conditions: 47, 110, 128, 130, 131, 134, 154, 155, 238

Add skeleton for: PQRS_Group_Dementia_0134 PQRS_Group_Diabetes_0126 PQRS_Group_Preventive_0431 PQRS_Group_RA_0337

Remove: PQRS_Group_Dementia_0285 PQRS_Group_Diabetes_0163 PQRS_Group_Preventive_0173 (I don't see files for this one.)

@leebc I thought we were going to maintain the 2015 rules, has this changed? I've been updating the code to allow for 2015 & 2016 processing, only a few modifications left.

As a side note: @aethelwulffe and I had discussed the clinical_rules table modifications to allow for rules that are viable for both 2015 & 2016, we can discuss this if the plan is still to provide both 2015 & 2016 processing.

It would basically meant creating a new directory structure for 2016, leaving 2015 in place.

For structural simplicity, having a directory for each years new measures would make sense. Additionally some discussion on how the table and code would be adjusted is in need.

Removed Group measures: PQRS_Group_Dementia_0285 PQRS_Group_Diabetes_0163

There is not code for PQRS_Group_Preventive_0173 (I don't see files for this one.)
