To implement the "Dispensation" and "Discard" feature we need to:
[x] fill out submitDispensation and cancelDispensation in NCP/openncp-national-connector/src/main/java/dk/openncp/nationalconnector/xdr/DocumentSubmit.java so that requests are forwarded to the country-a-service (#11)
[x] add controllers in country-a-service for the dispensation/discard endpoints (#13)
[x] Map dispensation CDA to StartEffectuationRequest (#14)
[x] Map dispensation CDA to CreatePharmacyEffectuationRequest (#15)
[ ] Map discard CDA to UndoEffectuationRequest (#16)
