When I apply for the API key, it comes up with the following questions.

What is your preferred protocol?

What is your preferred output format?

What answers should I choose?

I hope Mike will comment on this, but reading forum.sipsorcery.com, I found
this post from Mike http://forum.sipsorcery.com/viewtopic.php?p=12023#p12023

He mentions using YQL to convert XML into JSON so I'm led to believe that you
want XML output. Therefore, my guess is that the first entry should be XML-RPC
and the second XML. That should present XML to Mike's Whitepages lookup code.

The protocol is REST and output format is XML.

I left those entries at the default values (might have been blank), and nothing
bad ever happenned. I think the protocol and format are specified in the
request string sent to Yahoo.

Indeed, the protocol and format are specified in the request and you can use
any supported protocol and format, regardless of what you entered in those
fields. Probably their marketing guys wanted to know which protocols and
formats are most popular and added these questions (instead of checking their
server's statistics) ;-)
