Ethereal Sorcerer is casting spells on itself, damaging itself. They also will not chase you when you LoS them or attempt to out of range them.

Nexus-Prince Shaffar's RP triggered while in the first bosses room, I couldn't see the yells but heard the voice lines. Due to this it didn't trigger in his actual room.

can confirm the Sorcerer LoS issue, they also spawn little fiends that do not despawn and they continue to cast. We just had 40 of them attack us after we wiped.
