[x] Finish #81 (merge code)
[x] Merge code for reading native Philips DICOM data
[x] Add interface option for reading native Philips data or pixel data when both available
[x] Figure out what last 32 bytes of Philips native data is
- appears to be just padding (all 0s)
[ ] Figure out how to deal with native xPlane data (@mr-martian should get IRB approval to work with these files).

Two new todos:

[x] The old code stored "FrameTime" and "NumberOfFrames" in the metadata file, but I have not implemented that in the new code
[ ] None of the display options are selected by default, so at startup no image appears. Perhaps there should be a default (store in metadata which one we used last time?)
