Fix PMD links #32

The previous way asusmed all selected codes were relevant facets for all returned datsets, which is not true. This should encode only the filter facets that returned matching observations for each dataset's link to pmd.

Fixed a buglet that put the whole dataset doc (not just the URI) into the link. Bit hard to see what's going on when the remote site is down so often. Generally speaking the current pattern of errors is 503 to queries (which shouldn't affect ook outside of the etl process) and (once stardog freezes) 500 on the home page. The "server error" json we saw here in response to the cube explore links was different and actually indicated the links were wrong.
