Reintroduce PMD link #54

Once we've got code selections, we can reintroduce PMD links.

The facet-match descriptions will need to include the dimensions along with the codes. The match descriptions will be grouped by codelist as this is needed in the cells, but it should be relatively straightforward to merge code selections across codelists.

For cases where the user has selected a codelist but no codes then no filter should be applied on the PMD cube explorer.

0ef603b reintroduces the dimension URIs to the facet-match-descriptions.

This is done since https://github.com/Swirrl/ook/commit/d97db12f9d0e39616e1244e3d2d4f7fa720c8f83 and updated in subsequent commits to use the data in the right format.
