Show comments on notes/fields in tooltip (title attribute) #122

Should be straightforward: after rendering a table name or field, set div.title 
= comment (avoids worrying about escaping string for attribute).

Should this be able to be disabled?

I'll give a shot at implementing when I get some time. Great work on this tool.
