Changed utilisation ratio to collateralisation, to match v0.5 of the whitepaper.

New fee function (distribution based on CI vs Copt/Cmax)
New issuance/burning functions, handled by IssuanceController
Havven Foundation class added (currently only holds issued nomins/ extra havvens, to allow copt to be calculated to a reasonable value)

TODOs:

Fix logic of centralbank, fee optimiser
Add settings to UI to allow for changing them from the frontend
