docs: cleanup migration instructions #185

Tweaks to migration process.

created during #184

Changes

changed formatting of some doc text
fixed doc missing a ToC link
added "Create a New Build Action" steps
removed unused example __init__.py

Because #184 works (and the fixes to it[^1] were not something the docs should have solved), I'm adding this new documentation in to help with the next migrations.

[^1]: It had an odd and undesirable manner of loading its own site.css that just could not be backwards-compatible with production. I change it to use the typical way of loading custom CSS, and then it worked fine.
