Clarify how `@hand` and `<handShift>` interact #2339

In 11.3.2.1 there is a sufficiently clear explanation of @hand and <handShift>, but at the Newcastle council meeting, council agreed that there could be greater clarity on how the two may need to interact in some occasions (the discussion started with issue #2247).
