transpile.xslt needs documentation and possibly more a:documentation output #37

We noticed today that valItem/gloss and /desc were not being processed to generate a:documentation elements in transpile.xslt. We have added that processing today, but we also note that many templates are still lacking documentation, and there may be other cases where a:documentation has been missed. Check and fix as necessary.
