Introduce new UI #23

[x] inject Form Builder
[x] adjust Form Builder features to current requirements
[x] combine new UI with schemas CRUD
[x] allow to add form control additional information
[x] allow to set encoding of form control
[x] allow to mark an attribute as PII
[x] allow to select field name from predefined BIT's list or set custom field name

@mitfik , the UI changes seems to be done

This resolves #14 , resolves #13 , resolves #16 and perhaps resolves #15 (I'd vote to omit the "overlay" word as it does not provide any value to the UI)
