User reports that namespace fixup correctly forbids xml prefix for attributes, not for elements #170

Since we're catching the section 2.3 prohibition for attributes, we should also do so for elements (also, possibly, for processing instructions).

It appears that the not only does said customer already actually use the 'xml' prefix for elements, they've done so for years. Remarkable incompetence, against which we appear to be entirely helpless. Allow the idiocy for elements and attributes.
