Issue

The questions below should be made into a single multi select field:

• Live Birth but the baby died the same day in previous delivery:
• Partial Placenta in previous delivery:
• Severe Hemorrhaging in previous delivery (>500 ml):
• Preeclampsia in previous pregnancy :
• Experienced convulsions in previous delivery:
• Experienced convulsions and resulted in becoming unconscious after delivery:
• Gestational Diabetes in previous pregnancy:
• Incomplete Cervix in previous pregnancy:

As such (in two columns with "None of the Above" below a horizontal rule):

Has the patient ever experienced these conditions during any previous pregnancy?

• Placenta Previa (NOTE: was called "Partial Placenta")
• Newborn death within 24 hours after delivery
