The questions below should be made into a single multi select field:

As such (in two columns with "None of the Above" below a horizontal rule): Does the patient have any of the following medical conditions?

• Diabetes Mellitus
• Cardiac Disorder
• Hypertension before pregnancy
• Renal Disorder
• Asthma
• None

Tasks

• Remove the existing fields
• Add the new question
• Create an update script that moves old data into new format (note some of the naming is slightly different but they should still map)
