R.User Story #10. Mediators can create a new case #535

Mediators can create a new case in their dashboard. At the bottom of the page. They should be able to add:

[ ] case #
[ ] parent 1 name
[ ] parent 1 first name
[ ] parent 1 last name
[ ] parent 1 email
[ ] parent 1 password
[ ] parent 2 name
[ ] parent 2 first name
[ ] parent 2 last name
[ ] parent 2 email
[ ] parent 2 password
[ ] child 1 name??????

When A new case is created, THREE database entries occur. One for the new case, and one for each new parent.

For FE:

[ ] form at the end of the dashboard with the inputs for case # and whatever we decide.
[ ] submit button
