chore(green-hosting): refactor green hosting plugin to comply with IF 0.7.1 interface #20

Impact Framework substantially changed its interface. This refactors the plugin according to the new interface. It also adapts the manifest example in the README of the plugin to the newest manifest format (no node config, different pipeline phases).
