add feedback and error report links #9

feedback is done at the end of the course (link on conver page, perhaps add a 'last module' page as well?)

error reporting...ideally make this an anonymous/easy process via GitHub, but if that is not possible, a MS form is fine

links should be on main page, as well as a small link or icon on every module, to make error reporting easy
