Fixes #131

Github is currently implementing new features to improve Pull Requests and code reviews. This Pull Request adds a template for new Pull Requests to follow. The goal is to make standard information available in a consistent way.

PULL_REQUEST_TEMPLATE.md, this file specifies how Pull Requests are formatted by default.
