updated notification + questionnaire

Notification: Notification now works with onload. When user get to the notification page, they will automatically be notified to check their checklist. As well as get notified every 5 minutes to check the checklist.

• [ ] Will need to connect page to the notification button on the homepage

Travel Questionnaire: Updated UI to look as close to the homepage as possible.

• [ ] Will need to add the menu bar to the top as is on the homepage.
