Just made this locally and it seems that you switched two chars:
The glyphs uniFB95.001
and uniFB93.001
(medi and fina) should be switched
from this fea code:
sub uni06AA.medi by uniFB95.001;
sub uni06AA.init by uniFB94.001;
sub uni06AA.fina by uniFB93.001;
sub uni06AA by uni06AF.001;
I get this result:
unicode says: FB92 ARABIC LETTER GAF ISOLATED FORM FB93 ARABIC LETTER GAF FINAL FORM FB94 ARABIC LETTER GAF INITIAL FORM FB95 ARABIC LETTER GAF MEDIAL FORM

There are a few characters which they don't have a Unicode value, that would be great if we add them as a style (they are really common specialy for Persian)
