A coach should be able to create a new Coaching report by filling it an online form.
The coaching report will contain the following information:
Date and time that coach made contact
The form of communication (email, text message or phone call)
Duration of the contact in minutes
If this was a full coaching session that took place, or just an attempt to contact the particpant.
An open field for comments about the interaction.
A Date and Time the coach intends to contact to participant again.
A flag saying this will be the last contact (Participant is leaving the study, doesn't want to be contacted again)
