Recognition Ratings #160

This should match the work originally done in R34. It should not use the javascript JSPsych system, but be simple html input.

Drop the lemon exercise from recognition ratings. Display all the text at once. Keep the fill in of the missing letter. Always ask a question afterwards. Remove the final page. Remove the score. Drop the title screen, as there is no picture. Place the title in all capitals above the story.
