Bethany wants these questions to be more visually easy to read (horizontal vs vertical), so she wants something like the tables we have in "completing stories-continuation" on R34. I made one using a similar format to what we have been using in the other forms... clearly it still needs more work, styling and such, but it looks like it easily works for mobile versions.

Mental History: Add required to “Other” only if checked.
Problems with the table I made for expectancy bias… It scrolls all the way to the next story with just the first answer (it has three different questions). Basically, it’s taking the three questions as if they were only 1 (e.g. If I click ‘Prefer not to answer’ in one of them it disables the three)
Evaluation: There are answers that sometimes won’t click. For example, the “Not at all” doesn’t click in the question that says ‘How likely would you be to recommend this web program to others with similar anxiety?'I thought it was because the “Prefer not to answer” was in the same line, but I changed it and it’s still happening. ‘Somewhat’ won’t click on ‘How helpful did you find the web program in reducing your anxiety?’ … more similar problems
