Compilation currently throws two PTX:ERRORs for repeated xmlids

    * PTX:ERROR: the @xml:id value "TI-intro" should be unique, but is authored 2 times.
    *            Results will be unpredictable, and likely incorrect.  Information on the locations follows:
    *              located within: "TI-intro" (xml:id)
    *              located within: "TI-intro" (xml:id)
    * PTX:ERROR: the @xml:id value "TI-readiness" should be unique, but is authored 2 times.
    *            Results will be unpredictable, and likely incorrect.  Information on the locations follows:
    *              located within: "TI-readiness" (xml:id), "Readiness Assurance" (title)
    *              located within: "TI-readiness" (xml:id), "Readiness Assurance" (title)
