operator mute issue #340

Operators who are manually or automatically op'd should not be mutable via the mute command or callvote mute.

Additionally, if Steam authentication is disabled, are IPs used instead of Steam IDs in listip? We didn't test this scenario to verify.

removevote should remove all instances (there was a duplicate result in listip and it only removed the first entry)
