Add all damage from All-Totem of the Master. #729

Only the fire damage was being converted to the item. Added all the other elements so they get converted.

Also removes handler for "Vakash" weapon as it actually substitutes for all shadowflame enchanted weapons from Fyrakk. Not just Vakash.
