Added directives to allow extracting JWT claims to the request/response headers.
Renamed the auth_jwt_extract_sub directive to auth_jwt_validate_sub since the above new directives handle half of what it was doing before. BREAKING CHANGE
Refactored and renamed a bunch of stuff to clean up the code and make the handler function shorter and easier to follow.
The best way to review this might be to look at the end result in the IDE of your choosing rather than viewing the diff.
