Change "stream" to "test" in the docs #184

The docs are still littered with references to the "stream". Change it to talking about the "test".

The concept of a "stream of results" has been changed to simply referring to the "test" as in the TestStart and TestEnd events and subtests.

This is a big issue, so you're welcome to submit it a piece at a time.

With @dunsmoreb's work in #236 that looks like it's done it.
