Consistent naming of add-on #25

Currently the add-on uses the visible names 'Flow Graph', 'Git Flow Graph', 'Git Flow' and 'Stash Git Flow Graph'. In addition it is using 'gfc' (git flow chart), 'gitflow', 'GitFlowGraph', 'git-flow-vis', 'GitFlowVisualize' as prefixes in code. The servlets use 'git-flow-graph' as url prefix.

Maybe we should decide on a more consistent naming convention :) I would like to propose we switch to "Git Flow", with "gf" as prefix. @Teun : what is your preference?

I sort of prefer the longer name: Git Flow Chart. Git Flow suggests more support of the whole flow. While that could be interesting for later, I'd prefer the limited name/promise.

So I'll try to consistently say Git Flow Chart and gfc.
